11. Does your teen express feelings of worthlessness or guilt?
12. Does your teen seem to feel that nothing is worth the effort?
13. Does your teen suffer from insomnia?
14. Does your teen ever have panic attacks?
15. Has your teen had hallucinations or delusions?
16. Has your teen abused drugs or alcohol?
17. Has your teen had thoughts of death or suicide?
